Dual Diagnosis Case Management Team
The team supports adults living with a dual diagnosis (developmental disability and mental health issues) through a supported independent living (SIL) program. Case Managers assist clients in recovery using the Ontario Common Assessment of Need (OCAN) tool. Along with assessment, Case Managers provide client centered support services that promote stabilization and independent living as mandated by the Ministry of Community and Social Services. The Case Manager carries out these duties as a member of a multi-disciplinary team within CMHA Haliburton, Kawartha, Pine Ridge. Individual progress is a result of collaboration with psychiatrists, family members and community partners, psychoeducational opportunities, crisis intervention and addiction supports. The Dual Diagnosis program develops and monitors measureable outcomes to ensure continuous quality improvement.
