The Community Support Worker provides assistance to the case managers as part of a team approach to consumer services. This service assists consumers with routine medical and psychiatric appointments and other routine services which allow the case manager to focus on other aspects of the consumer’s needs. The Community Support Worker also engages consumers in identifying areas of need related to independent living skill acquisition. This includes an independent living assessment and formulating a plan with the client which will assist in developing skills to benefit independent living. The Community Support Worker also works with all consumers transitioning from formal case management towards final discharge for 90 days as part of case management discharge process. There is also a monthly follow-up for 12 months post discharge which ensures consumers continue to maintain a stable lifestyle after formal case management services are completed.
