The Community Support Worker (C.S.W.) is responsible for providing flexible supports for persons living with mental illness, a concurrent disorder, and/or a dual diagnosis. The C.S.W. carries out duties as a member of an inter-disciplinary mental health team and may also provide support to other community support service providers. The role is integral to supporting people who are in the C.M.H.A. H.K.P.R. Supportive Housing program.
The C.S.W. provides individualized support and intervention to clients in the community. They promote successful independent living using evidenced based interventions.
Support can include completing paperwork for Ontario Works, O.D.S.P. applications, support to medical appointments and completing referrals on behalf of the client. Supports are identified based on completion of an Intake assessment and the identification of goals. C.S.W. supports are brief, not long-term.
